Pharmacists Play Essential Roles in Transitions of Care

Pharmacists and pharmacy technicians are perfectly positioned to help manage patients’ transitions of care, according to an interview with Lindsey Clark, PharmD, MBA, and Brandon Harkonen, PharmD, MS, both from Michigan Medicine.

In the interview, Clark said pharmacists work within several programs handling transitions of care, including in the anticoagulation discharge program, in the outpatient centralized transitions of care service, and in the transplant program. Harkonen added that pharmacists are also providing their independent assessments of therapy in each of these programs, demonstrating their value and ensuring the highest quality of care.
